Please make sure to review my policies to understand your rights and responsibilities. You will receive a confirmation e-mail for your appointments and also whenever any appointment changes are made. Please always make sure to read them to make sure everything is correct.

Please take a moment to fill out and print my intake and policy form. Please be sure to bring these forms with you on your first appointment.

Please Note: If you have previously filled out the forms and had a Massage with Misty you probably already have an account set up and you will need to call Misty to get your username and password.

If you do not have a printer or you forget to bring them; please make sure to arrive 10 minutes early to read, fill and sign the forms.
